The index card relates to Applications for Financial Assistance - Not Exceeding £100 - for Purpose of Re-instating Dwelling Damaged by Earthquake forms. At the top of the index card the Owner's name and address they occupy as well as the Property House number or name that the application is filed in relation to, as well as the application number. The middle section of the card lists details of the nature of the work authorized, including work to Chimneys, Tanks and Stands, Plumbing, Drains, Lighting, Carpentry, Glazing and Other Repairs. The name of the contractor who completed the work and the amount authorised to complete the work is listed, as well as any payments made.
