The instructions and regulations are on the front cover, the inside front cover and inside the rear cover. These booklets are For Issue to Female Persons Only and are marked with a large W. (a) Includes a certificate of registration ( No 666367 ) signed by the Postmaster and with the information of the registered holder. It is also stamped with the date 5 June 1943. After this page are quarter-page receipts where the perforated payment slip has been removed, These all carry the same serial number and are stamped with the date of payment, either in 1943 or 1944. These receipts alternate with full pages that all explain the rules of the Social Security Act of 1938 to the effect that during May all persons over 16 years of age must furnish proof of income for the previous year, whether or not they are exempt from paying the social security fee. In this booklet Miss Baker is a student living in Wairoa.

After this page is a perforated half-page with the rules of the Social Security Act of 1938 in that during May all persons over 16 years of age must furnish proof of income for the previous year, whether or not they are exempt from paying the social security fee. Following that, there is a quarter-page receipt for payment made that is stamped on 29 May 1945. The payment coupon that was attached has been removed. All the remaining perforated payment coupon and receipt pages are intact. The three quarter-page part that was to be removed is titled Registration Fee and includes the registration number, the dates due (the 1 of May between 1946 and 1950). Spaces for information of the contributor, and a date-stamp are included. The final quarter page is perforated again and repeats the registration number, payment due date and has room for the date stamp. The last page is instructions and paperwork for acquiring the third series of the booklet. In this booklet Miss Baker is a student living in Napier.
